As we age, repeated facial expressions combined with natural collagen loss create deep lines that remain visible even at rest. Nasolabial folds running from the nose to the mouth, marionette lines extending downward from the corners of the lips, forehead creases, and crow’s feet around the eyes are among the most common signs of facial ageing.
At Cambridge Clear Beauty, we’ll take the time to assess your skin and figure out the right treatment approach. What’s suitable can depend on factors such as the depth and location of the lines and your skin quality. We’ll explain which options are appropriate for your particular concerns and what you can realistically expect from each approach.
What are Deep Facial Wrinkles?
Deep facial wrinkles are lines and folds that have become etched into the skin over time, remaining visible even when your face is completely relaxed. Unlike fine lines that may only appear with certain expressions, deep wrinkles represent a more advanced form of skin ageing where the underlying structure has been permanently altered.
Common types of deep facial wrinkles include:
- Forehead wrinkles: Horizontal lines across the forehead
- Frown lines: Vertical lines between the eyebrows
- Crow’s feet: Lines radiating from the outer corners of the eyes
- Under-eye wrinkles: Fine to deep lines beneath the lower eyelid
- Nasolabial folds: Lines running from the nose to the corners of the mouth
- Marionette lines: Lines extending downward from the corner of the mouth towards the chin
- Lip wrinkles: Vertical lines around the mouth, sometimes called smoker's lines
The severity of deep wrinkles varies considerably. Some people develop moderate lines in their forties, whilst others may not see significant wrinkling until much later. Genetics, sun exposure and smoking all influence how quickly and severely these wrinkles develop.
What Causes Deep Facial Wrinkles?
Deep facial wrinkles develop through a combination of repetitive muscle movement, natural ageing processes and cumulative environmental damage that breaks down the skin’s structural integrity over time.
Repeated Facial Expressions and Muscle Activity
Every time you smile, frown, squint or raise your eyebrows, the underlying facial muscles contract and the skin folds in the same pattern. In younger skin with healthy collagen reserves, these lines disappear once the expression relaxes.
After years of repeated folding in the same places, however, the skin loses its ability to spring back, and the creases become permanently etched in. This is why wrinkles tend to form in predictable patterns that mirror our most frequent expressions.
Natural Ageing and Collagen Breakdown
As we age, the body’s production of collagen and elastin begins to decline. This process typically starts in the late twenties and accelerates significantly after 40.
At the same time, the skin becomes thinner and drier, losing the cushioning and moisture that once helped it resist creasing. The combination of reduced structural support and repeated mechanical stress from facial movement results in deep wrinkles that form and deepen over the years.
Sun Damage and Environmental Factors
Ultraviolet radiation from sun exposure is one of the most significant accelerators of deep wrinkling. UV damage breaks down collagen fibres at the cellular level, a process known as photoageing, weakening the skin’s ability to resist fold formation.
Smoking has a similarly destructive effect, reducing blood flow to the skin and introducing toxins that impair collagen synthesis and repair. Dehydration, poor nutrition and exposure to pollution further compromise skin quality and resilience, making wrinkles appear earlier.
Genetics and Skin Type
Individual susceptibility to deep wrinkles varies widely due to genetic factors. Some people are born with naturally thicker, more collagen-rich skin that resists wrinkling longer, whilst others have thinner or more delicate skin that shows signs of ageing earlier.
Skin tone also plays a role; fair skin tends to wrinkle more readily than darker skin types due to lower melanin protection against UV damage. Even with identical sun exposure and lifestyle habits, however, two people may develop wrinkles at very different rates.
Should I Consider Treatment for Deep Facial Wrinkles?
The decision to treat deep facial wrinkles is entirely personal. What matters is whether the lines are bothering you, affecting how you feel about your appearance or making you look older or more tired than you actually feel.
When Wrinkles Become Bothersome
Many people seek treatment when deep wrinkles start to feel permanent or when they notice lines appearing even without any facial expression. This might be when forehead creases no longer smooth out, when nasolabial folds deepen noticeably or when crow’s feet become more prominent.
When Multiple Concerns are Present
Deep wrinkles rarely exist in isolation. Often, they’re accompanied by other changes, such as volume loss, skin texture issues, or pigmentation damage. In these cases, treating the wrinkles alone may not yield the results you’re hoping for.
Understanding how different concerns interact and which combination of treatments might work together can make the difference between subtle improvement and meaningful change.
Realistic Expectations
Deep wrinkles that have been present for years won’t vanish completely with any treatment. What’s achievable depends on factors like skin thickness, wrinkle depth and how much structural damage has occurred.
Treatment Options for Deep Facial Wrinkles at Cambridge Clear Beauty
Non-Surgical Treatments
The appropriate non-surgical approach depends on whether your wrinkles are primarily caused by muscle movement, loss of underlying volume, or cumulative skin damage.
Dermal Fillers
Hyaluronic acid-based dermal fillers are designed to restore volume and soften deeper static wrinkles, including nasolabial folds, marionette lines and lip lines. Fillers work by gently lifting the crease from beneath the skin, reducing its depth and restoring more youthful contours.
With an experienced clinician, someone who knows the detailed anatomy of the face, fillers can also be judiciously placed not only to fill surface contours but also more deeply, to restore loss of bony support of the soft tissues. With more expert knowledge of the facial fat pads and other structures it may be possible to achieve a modest lifting effect and sometimes to restore some youthful features that have been lost. However a natural ‘untreated’ outcome will always be our aim.
LaserMe Skin Resurfacing
This non ablative fractional resurfacing laser treatment targets damaged surface layers of skin while stimulating collagen remodelling at deeper levels. It is effective for softening a range of wrinkles simultaneously, refining texture, evening tone and improving fine to moderate lines across the face.
Chemical Peels
Medical-grade chemical peels exfoliate the outer layers of damaged skin, promoting the growth of fresh, smoother skin. They can be categorised as ‘nutritive’ to the skin such as fruit acid (glycolic acid) and retinol peels as well as resurfacing peels such as TCA (trichloroacetic acid). Light to medium peels can soften fine lines and improve surface texture, while deeper peels may help reduce more established wrinkles and pigmentation irregularities.
Morpheus8
Morpheus8 uses radiofrequency-assisted microneedling to heat the tissue beneath the skin and stimulate collagen production. This treatment can improve moderate wrinkles, enhance firmness and refine texture, particularly in areas where skin quality has declined due to age or sun exposure.
Anti-Wrinkle Injections
Considered the gold standard for dynamic wrinkles, anti-wrinkle injections temporarily relax targeted facial muscles responsible for expression lines such as forehead lines, frown lines and crow’s feet. By reducing muscle activity, the overlying skin appears smoother and more refreshed.
Surgical Treatments
For deeper wrinkles that have not responded to non-surgical approaches, surgical correction may be necessary.
Full Face Lift, Neck Lift, Face & Neck Lift
A full facelift addresses deeper wrinkles in the mid and lower face by tightening the underlying SMAS layer, repositioning descended facial tissues and removing excess skin to restore structural support. Every face lift involves a bit of a neck lift and every neck lift involves a bit of a face lift – this is needed to maintain harmony in the appearance and so to look natural, ‘unoperated’; in a face and neck lift all areas can be addressed. This comprehensive approach smooths pronounced folds, improves sagging along the jawline and enhances overall face and neck definition.
Eyelid Surgery (Blepharoplasty)
Specifically treats deep wrinkles, crepiness and excess skin around the eyes, as well as unsightly ‘bags’ due to protrusion of the fat pads behind the eyelids. Can be performed on the upper lids, lower lids or both, depending on where wrinkling is most pronounced. Often combined with other facial or medical aesthetic procedures for comprehensive rejuvenation.

Frequently Asked Questions
How to get rid of wrinkles?
The most effective approach depends on what’s causing your wrinkles. Dynamic lines from muscle movement (forehead, frown lines, crow’s feet) respond well to anti-wrinkle injections. Deep static folds, such as nasolabial lines, typically require dermal fillers or laser resurfacing. Very deep, established wrinkles often need surgical correction, particularly when accompanied by loose skin.
Can deep wrinkles be removed completely?
Deep wrinkles can be significantly improved but rarely disappear entirely. The goal is noticeable softening and smoothing, not complete erasure. The amount of improvement possible depends on wrinkle depth, skin thickness, duration and cause.
What is the most effective treatment for deep wrinkles?
There’s no single answer because different wrinkles require different approaches. For many people, combining treatments delivers the best results. Treatment recommendations should be based on careful assessment of your individual wrinkle pattern and skin quality, not a one-size-fits-all approach.
How long do wrinkle treatments last?
Duration varies by treatment type. Anti-wrinkle injections last 3-6 months, dermal fillers last 12-18 months and laser resurfacing results can last several years with proper sun protection. Surgical treatments like facelifts provide the longest correction, lasting 10+ years.
