I have lost two close relatives to malignant melanoma. I was referred on by my GP for ‘ urgent skin cancer assessment’ of a suspicious mark on my face. NHS. It was a two week wait for a photo then a predicted 3 week wait for a result sent to me by text. With fair Scots skin and family history, l struggled with what would be a 5 week wait just for a diagnosis.

I made an appointment at the clinic. I got an appointment and a result within days. Thankfully negative. The result was told to me by a person, not the text message the NHS promised. I also got skin care advice and a prescription for previously undetected sun damage which l greatly appreciate. I was treated with utmost kindness and courtesy. I am so grateful that l heard your radio advert offering this service.

I underwent a rhinoplasty surgery with Mr Ahmad over 3 years ago.

Preparation for the surgery could have been more thorough. I was unaware of extent of recovery timeline and importantly, what this could look like. I had swelling for well over two years and even now the tip is still refining. I understand now how complex a surgery this is.

In hindsight I wish Mr Ahmad, like the majority of other surgeons as I understand, had provided an image or drawing of what he had in mind prior to the surgery.

The early recovery was hard, I had a lot of swelling, bruising, and looked completely unrecognisable. This was somewhat expected though did seem at the more severe end of the scale. The surgery took 4.5 hours.

Following the surgery I was left with an asymmetrical residual hump, depression through centre of nose, retracted nostrils and a large scar on one nostril through error of Mr Ahmad as I understand, though he was never forthcoming in explaining reasoning. I felt it had a ‘done’ look and could have been more natural. After the surgery, as I was not entirely happy with the result given the above, and ironically at this stage and not before, Mr Ahmad suggested therapy or that I may have body dysmorphia, to correct the relationship with my nose. Therapy was not offered prior to the surgery of course. I felt very much gaslit and on meeting with other practitioners concerns were understood and addressed.

Another insensitive moment came when I mentioned perhaps spending too much time on Instagram, where content served to me is focused on appearance. Mr Ahmad replied they liked Instagram, alluding to it’s how they make their money.

Mr Ahmad offered to correct the scar on my nostril, which improved it somewhat though not entirely it is still visible. Ironically during the procedure Mr Ahmad nearly nipped my lip with the scissors, much to the nurse’s surprise. Not the comforting attention to detail of a surgeon.

i had my breast reduction surgery last year and just had my last appointment today! I've been seeing Mr Ahmad amd his staff at Cambridge Clear Beauty for four years now for various treatments and precedures.
I've had three deep face peels, scar revision on some deep acne scars and skin consultations with him and his staff and my skin now looks the best it has for years after suffering from sun damaged, acne prone skin for a long time I never imagined my skin could look so good.

I had a septorhinoplasty with Mr Ahmad to correct a post-traumatic deviated septum. I had undergone surgery when I was younger, but it hadn’t fully resolved the breathing issues in one nostril, and I was also self-conscious about my nose profile.

From the very beginning, Mr Ahmad took the time to listen carefully and make detailed notes, even producing incredibly precise drawings to capture what we discussed. His insights into what would suit my face best were invaluable, and it’s clear that his experience and knowledge guided the outcome.

I had my top eyelids trimmed back and my bottom eyebags removed.
I did have extensive bruising that was explained by Mr Ahmad would happen. My top eyelids healed fairly quickly but the bottom eyebags are more difficult. Mr Ahmad and Nikki have always answered my questions and have always seen me regularly or when iv had a worry and eased my concerns.
Overall, my eyes are healing now, and after 3 months are doing well and looking much better. Mr Ahmad told me it could take up to a year for my healing process to end.
As it is an invasive surgery I can understand why, but thankfully its getting there and I'm pleased with how my results are starting to look.

Bags under the eyes are areas of puffiness, swelling or loose tissue that form beneath the lower eyelids, creating a tired, aged or heavy appearance. They appear as bulges or pouches of tissue beneath the eyes, often accompanied by dark shadows, hollowing or loose, wrinkled skin.

In mild cases, under-eye bags may appear as subtle puffiness or a slightly swollen look, particularly in the morning. In more advanced cases, they create pronounced bulges, deep tear troughs or festoons (cheek bags) that significantly alter the eye area and make the face look perpetually exhausted regardless of rest or sleep quality.

What Causes Bags Under the Eyes?

Under-eye bags result from a combination of structural weakening, fat pad displacement, fluid retention and skin laxity that occur with age and are exacerbated by genetic and lifestyle factors.

Natural Ageing and Orbital Fat Herniation

As we age, the orbital septum, a thin membrane that holds fat pads in place behind the eye, naturally weakens and stretches. When this barrier loses integrity, the fat that cushions the eye begins to push forward and bulge beneath the lower eyelid, creating the characteristic puffy appearance of under-eye bags. At the same time, collagen and elastin in the lower eyelid skin decline, causing the skin to thin, stretch and sag, making the protruding fat even more visible.

Fluid Retention

The delicate tissue beneath the eyes is particularly prone to fluid accumulation, especially during sleep, when lying flat allows fluid to pool in the lower eyelid area. Factors such as high salt intake, hormonal changes, dehydration and certain medications can all increase fluid retention, making under-eye bags appear more pronounced, particularly in the morning.

Weakening of the Lower Eyelid

The muscles and ligaments that support the lower eyelid gradually lose tone with age, allowing the eyelid to pull away from the eye slightly and creating a space where fat can bulge forward. The combination of weakened support structures and displaced fat creates the appearance of heavy, baggy lower lids.

Genetics and Skin Quality

Genetic factors play a significant role in determining who develops under-eye bags and when they appear. Some people inherit naturally thinner lower eyelid skin, weaker orbital septa, or a tendency toward fat pad protrusion, which can cause bags to form in their twenties or thirties.

Allergies and Chronic Inflammation

Chronic allergies, sinus issues and conditions that cause repeated inflammation around the eyes can contribute to persistent puffiness and gradual stretching of the lower eyelid tissue. Frequent eye rubbing due to allergies or irritation weakens the delicate skin and can damage the orbital septum, accelerating fat herniation.

Sleep, Lifestyle and Dietary Habits

Chronic sleep deprivation, sleeping flat without head elevation, excessive alcohol consumption and high sodium intake all contribute to fluid retention and worsen the appearance of under-eye bags. Lack of sleep also causes blood vessels beneath the eyes to dilate, creating dark circles that make bags more noticeable.

Sun Damage and Environmental Factors

UV radiation breaks down collagen and elastin in the thin lower eyelid skin, leading to photoageing, premature sagging and loss of elasticity. The under-eye area is often inadequately protected by sunscreen, making it particularly vulnerable to sun damage.

Smoking reduces blood flow to the delicate periorbital tissue and impairs collagen production, whilst environmental pollutants and dehydration further compromise skin quality, leading to under-eye bags appearing earlier and more severely.

Should I Consider Treatment for Bags Under the Eyes?

The decision to treat under-eye bags depends on whether they bother you, how they affect your appearance and confidence and what improvement you’re looking for.

When It Affects Your Confidence

Many people consider treatment when under-eye bags create a perpetually exhausted appearance that doesn’t match how they feel. If people frequently ask if you’re tired or unwell, if the bags are visible even after a full night’s sleep, or if you constantly look older or more worn out than your actual age, these are valid reasons to seek treatment.

When They’re Worsening Over Time

Under-eye bags typically don’t improve on their own and tend to worsen progressively as the orbital septum continues weakening and more fat herniates forward. If you’ve noticed the bags becoming more pronounced over recent years, earlier treatment often delivers better results than waiting until the stretching becomes more severe.

Setting Realistic Expectations

Lower blepharoplasty can remove or reposition herniated fat and tighten loose skin, creating a smoother, more refreshed under-eye area. However, it won’t eliminate crow’s feet, change your fundamental eye shape or address dark pigmentation.

Lower Eyelid Surgery (Lower Blepharoplasty)

Lower blepharoplasty removes or repositions herniated orbital fat and tightens loose, stretched skin beneath the eyes, creating a smooth, refreshed under-eye contour. The procedure can be performed using two different approaches depending on your specific concerns.

Transconjunctival Approach

When under-eye bags are primarily due to fat herniation with minimal excess skin, the incision is made inside the lower eyelid (through the conjunctiva), leaving no visible external scar. This technique allows the surgeon to remove or reposition bulging fat whilst preserving the lower eyelid skin. It’s most appropriate for younger patients or those with good skin elasticity.

Transcutaneous Approach

When both fat herniation and significant skin laxity are present, the incision is made just beneath the lower lash line. This approach allows removal or repositioning of fat alongside excision of excess, loose skin and tightening of the lower eyelid. The incision heals along the natural crease beneath the lashes, making it virtually imperceptible once fully healed.

In some cases, fat repositioning rather than removal is performed. Instead of simply excising the herniated fat, it’s redistributed into the tear trough area (the hollow beneath the bag) to create a smoother transition and eliminate both the bulge and the depression simultaneously. This technique prevents the hollowed appearance that can occur with aggressive fat removal.

How to get rid of bags under the eyes?

The only effective way to eliminate under-eye bags is through lower blepharoplasty, a surgical procedure that removes or repositions herniated orbital fat and tightens excess skin. Lifestyle modifications like reducing salt intake, elevating your head while sleeping and managing allergies may temporarily reduce fluid-related puffiness, but cannot reverse established bags caused by fat herniation.

Under-eye bags are physical bulges caused by herniated orbital fat pushing forward beneath the lower eyelid. Dark circles are pigmentation or visible blood vessels beneath the thin under-eye skin, creating shadowing or discolouration without actual tissue bulging. The two conditions often coexist but require different treatments.

Under-eye bags caused by fat herniation and structural weakening cannot be entirely prevented, as they result from natural ageing processes and genetic predisposition. However, their severity can be minimised through consistent sun protection, avoiding smoking, managing chronic allergies, limiting eye rubbing and maintaining a stable weight.
