I have lost two close relatives to malignant melanoma. I was referred on by my GP for ‘ urgent skin cancer assessment’ of a suspicious mark on my face. NHS. It was a two week wait for a photo then a predicted 3 week wait for a result sent to me by text. With fair Scots skin and family history, l struggled with what would be a 5 week wait just for a diagnosis.
I made an appointment at the clinic. I got an appointment and a result within days. Thankfully negative. The result was told to me by a person, not the text message the NHS promised. I also got skin care advice and a prescription for previously undetected sun damage which l greatly appreciate. I was treated with utmost kindness and courtesy. I am so grateful that l heard your radio advert offering this service.

Preparation for the surgery could have been more thorough. I was unaware of extent of recovery timeline and importantly, what this could look like. I had swelling for well over two years and even now the tip is still refining. I understand now how complex a surgery this is.
In hindsight I wish Mr Ahmad, like the majority of other surgeons as I understand, had provided an image or drawing of what he had in mind prior to the surgery.
The early recovery was hard, I had a lot of swelling, bruising, and looked completely unrecognisable. This was somewhat expected though did seem at the more severe end of the scale. The surgery took 4.5 hours.
Following the surgery I was left with an asymmetrical residual hump, depression through centre of nose, retracted nostrils and a large scar on one nostril through error of Mr Ahmad as I understand, though he was never forthcoming in explaining reasoning. I felt it had a ‘done’ look and could have been more natural. After the surgery, as I was not entirely happy with the result given the above, and ironically at this stage and not before, Mr Ahmad suggested therapy or that I may have body dysmorphia, to correct the relationship with my nose. Therapy was not offered prior to the surgery of course. I felt very much gaslit and on meeting with other practitioners concerns were understood and addressed.
Another insensitive moment came when I mentioned perhaps spending too much time on Instagram, where content served to me is focused on appearance. Mr Ahmad replied they liked Instagram, alluding to it’s how they make their money.
Mr Ahmad offered to correct the scar on my nostril, which improved it somewhat though not entirely it is still visible. Ironically during the procedure Mr Ahmad nearly nipped my lip with the scissors, much to the nurse’s surprise. Not the comforting attention to detail of a surgeon.

I did have extensive bruising that was explained by Mr Ahmad would happen. My top eyelids healed fairly quickly but the bottom eyebags are more difficult. Mr Ahmad and Nikki have always answered my questions and have always seen me regularly or when iv had a worry and eased my concerns.
Overall, my eyes are healing now, and after 3 months are doing well and looking much better. Mr Ahmad told me it could take up to a year for my healing process to end.

What are Deep Facial Wrinkles?
Deep facial wrinkles are lines and folds that have become etched into the skin over time, remaining visible even when your face is completely relaxed. Unlike fine lines that may only appear with certain expressions, deep wrinkles represent a more advanced form of skin ageing where the underlying structure has been permanently altered.

What Causes Deep Facial Wrinkles?
Deep facial wrinkles develop through a combination of repetitive muscle movement, natural ageing processes and cumulative environmental damage that breaks down the skin’s structural integrity over time.
Repeated Facial Expressions and Muscle Activity
Every time you smile, frown, squint or raise your eyebrows, the underlying facial muscles contract and the skin folds in the same pattern. In younger skin with healthy collagen reserves, these lines disappear once the expression relaxes.
After years of repeated folding in the same places, however, the skin loses its ability to spring back, and the creases become permanently etched in. This is why wrinkles tend to form in predictable patterns that mirror our most frequent expressions.
Natural Ageing and Collagen Breakdown
As we age, the body’s production of collagen and elastin begins to decline. This process typically starts in the late twenties and accelerates significantly after 40.
At the same time, the skin becomes thinner and drier, losing the cushioning and moisture that once helped it resist creasing. The combination of reduced structural support and repeated mechanical stress from facial movement results in deep wrinkles that form and deepen over the years.
Sun Damage and Environmental Factors
Ultraviolet radiation from sun exposure is one of the most significant accelerators of deep wrinkling. UV damage breaks down collagen fibres at the cellular level, a process known as photoageing, weakening the skin’s ability to resist fold formation.

Should I Consider Treatment for Deep Facial Wrinkles?

Non-Surgical Treatments
The appropriate non-surgical approach depends on whether your wrinkles are primarily caused by muscle movement, loss of underlying volume, or cumulative skin damage.
Dermal Fillers
Hyaluronic acid-based dermal fillers are designed to restore volume and soften deeper static wrinkles, including nasolabial folds, marionette lines and lip lines. Fillers work by gently lifting the crease from beneath the skin, reducing its depth and restoring more youthful contours.
With an experienced clinician, someone who knows the detailed anatomy of the face, fillers can also be judiciously placed not only to fill surface contours but also more deeply, to restore loss of bony support of the soft tissues. With more expert knowledge of the facial fat pads and other structures it may be possible to achieve a modest lifting effect and sometimes to restore some youthful features that have been lost. However a natural ‘untreated’ outcome will always be our aim.
LaserMe Skin Resurfacing
This non ablative fractional resurfacing laser treatment targets damaged surface layers of skin while stimulating collagen remodelling at deeper levels. It is effective for softening a range of wrinkles simultaneously, refining texture, evening tone and improving fine to moderate lines across the face.
Chemical Peels
Medical-grade chemical peels exfoliate the outer layers of damaged skin, promoting the growth of fresh, smoother skin. They can be categorised as ‘nutritive’ to the skin such as fruit acid (glycolic acid) and retinol peels as well as resurfacing peels such as TCA (trichloroacetic acid). Light to medium peels can soften fine lines and improve surface texture, while deeper peels may help reduce more established wrinkles and pigmentation irregularities.
Morpheus8
Morpheus8 uses radiofrequency-assisted microneedling to heat the tissue beneath the skin and stimulate collagen production. This treatment can improve moderate wrinkles, enhance firmness and refine texture, particularly in areas where skin quality has declined due to age or sun exposure.
